Permission forms, event forms, registration forms. Completed forms may be turned in to Uma or the church office.
Permission Form Parent permission form to be used by youth & parents for activities as requested by Youth Director.
Emergency Information Form One Emergency Information Form needs to be filled out for each youth. This information will be helpful to the youth leaders as they attend activities, at Trinity and off site, with the youth.
